Blanks can still be dangerous. Maybe he was very close to them. Seems very odd that there were two victims though.

Do you know what a blank is? A blank has no bullet, its just a casing which fires a bit of gunpowder. At very close range you can damage an ear, eye or get some burns, it doesnt kill you. I've shot blanks at colleagues when in the army. The gun has a stop screwed on it to create the pressure in the barrel for the rifle to work and recoil. Movie prop guns have the barrel welded shut on the inside to make it look real, if you would fire a real bullet from a prop gun, the gun would explode. So there must have been a mix up with a working gun.

Everyone seems to be fixating on the two victims line as if this implicated Alec as a deliberate perpetrator of a crime.

As stated before, it sounds like a bullet got lodged in the chamber and the following round (presumably a blank) caused the lodged bullet to leave the chamber.

As someone mentioned above, there may well have been protective welding inside the chamber - though, not sufficient to stop the bullet, evidently - the reaction of which to the bullet causes a miniature explosion, shrapnel, etc. thereby damaging - though not fatally - the second victim.

The two victims were the director and the director of photography / cinematography and the scene was supposedly Alec facing down the lens of the camera, so they’d like both be stood / sat right behind the camera.
