If England play with two holding midfielders then Gerrard should be ok for 90 mins. They'd be inviting the opposition on to them so Gerrard can pick his runs every now and then on the counter.

Well Casillas has been captain for Spain and it doesn't do them any harm, although who's captain for them likely has little importance, and not just cause they're such a good team. Most other countries don't see captaincy as such a big deal. It's more a big deal for us cause of the media surrounding it.
