Portugal beat Denmark, Denmark beat Holland and Holland beat Portugal (in this scenario, assuming Holland are on positive goal difference to the others) meaning no team has a better head to head than the other, so it has to go down to goal difference.

I think that's it anyway.

The head to head will not count as there will be 3 teams on 3 points. I think if Holland win by 2+ goals will advance.

yea if 3 teams have the same points its goal diff...only if 2 teams on same point it goes head to head........

It's not just goal difference though. It's goal difference between the three tied teams.

A Germany win and a 2+ goals victory for the Dutch and they're through

If all games finished 0-0, for example, the teams with the higher co-efficient will go through.

So if England beat Ukraine 2-0, and France beat Sweden 3-2 we will finish higher than France due to our higher co-efficiency ranking.

No jokes. Even unfairer than drawing lots.

The only time this wouldn't be the case is if both teams are playing each other. Then it would be a penalty shoot out.
